  Were you a citizen by naturalization of Transkei, Bophuthatswana, Venda or Ciskei before   October 6, 1995?
  Were you granted a certificate of naturalization on or after October 6, 1995?
  Are you the spouse/widow(er) of a South African citizen and a permanent resident of
  South Africa and have lived in South Africa for 2 or more years back from today's date and
